An overview of the general health status of community residents and a focus on the specific health care needs of special populations.

The North Penn Community Health Needs Assessment was conducted in two phases. Phase I, conducted in the fall 2002, provides an overview of the general health status of community residents. Phase II, conducted in the winter 2002/2003, provides a more focused examination of the special healthcare needs of community residents who are seriously mentally ill; mentally retarded or developmentally disabled; physically challenged; or have cancer or a substance abuse condition.
